How to prepare

Placing a loved one in an assisted living facility is never easy—it’s a deeply personal and emotional decision. However, taking the right steps in advance can ease the transition and ensure their well-being.

One of the first steps is completing the LIC 602A form, which must be filled out by a physician familiar with your loved one’s medical history. This form includes a tuberculosis (TB) test requirement and must be signed by the doctor. Once completed, it remains valid for one year.
